18.27 Needs Assessment: A Comprehensive Mentorship Program for Preliminary Resident Surgical Trainees

Introduction:
Non-designated preliminary surgical residents (PSR) encounter significant challenges in securing positions that lead to the completion of residency training. Evidence suggests that PSR may require more support, particularly in mentoring, compared to their colleagues in categorical positions. However, few studies have investigated the role of mentorship in the PSR experience and its impact on career outcomes. This study serves as a needs assessment of non-designated PSR at a single institution to identify areas of need and develop a formal, structured mentorship program for PSR.

Methods:
PSR within the department of surgery at a single program participated in a voluntary needs assessment survey from 2023-2024. Information collected included demographics, specialties applied to, and experiences and perceptions regarding guidance and mentorship received prior to applying. At the end of the year, a post-reapplication experience survey was administered, and respondents were surveyed regarding their overall application experience and success in matching. 

Results:
Of the 18 total PSR, 8 (5 PGY-1s and 3 PGY-2s) completed the pre-reapplication survey, and 8 (6 PGY-1s and 2 PGY-3s) completed the post-reapplication survey (44% response rate). A majority of respondents indicated that guidance from a mentor would have been helpful; however, less than 50% reported having an available mentor or receiving guidance on applying prior to their preliminary year. Among those who met with a faculty mentor during their preliminary year, 88% had fewer than 3 meetings over a 9-month period. Both pre- and post-reapplication, respondents most commonly reported experiencing emotions of anxiety, fear, sadness, and loss of confidence. Of the post-reapplication survey respondents, 38% successfully enrolled in categorical positions through Match week.

Conclusion:
There is an unmet need for a comprehensive mentorship program among PSR seeking categorical training positions. While meetings with faculty were helpful during the preliminary year, additional structured interventions, such as required meetings, practice interviews, and networking opportunities, may be necessary to increase engagement with mentoring resources and improve matriculation outcomes.
